HOME HEALTH AGENCY Skilled Nursing Registered Nurse (RN) - An RN will make the first visit to your home and be responsible for teaching you about medication, treatment, your disease and any modifications you may need to make to manage your disease. Licensed Vocational Nurse (LVN) - An LVN may perform some of the same duties as the Registered Nurse and is supervised by the Registered Nurse.
Physical Therapist (PT) - A PT will perform gait training, set up a home exercise program and plan treatment intended to restore and preserve function. Physical Therapy Assistant (PTA) - A PTA may perform some of the same duties as the Physical Therapist and is supervised by the Physical Therapist.
Speech Language Pathologist (SLP) - An SLP will assist in recovering swallowing, speaking, listening, reading and writing skills. Therapy may involve teaching new ways to safely swallow and communicate.
Occupational Therapist (OT) - An OT will guide and instruct you in caring for yourself and activities of daily living to improve your independent functioning. Occupational Therapy Assistant (OTA) - An OTA may perform the therapy plan according to the directions of the Occupational Therapist.
Medical Social Worker (MSW) - An MSW will counsel patients and family, plan for continued service after discharge and act as liaisons with other community agencies.
Certified Nursing Assistant (CNA) - A CNA will provide assistance with personal care until the patient and or caregiver is independent and able to independently perform this care. Services include Bathing, Dressing, Cleaning, Laundry, Meal Preparation, Escort, Shopping and Assisting with Medication.
